For me... it would be Braindead, Bad taste, Requiem for a dream and maybe Lost Highway. :D I tried to watch Begotten and Eraserhead, but these were just too surreal xD

Donnie Darko, Prestige, Memento, Requiem for a dream, Inception, Clockwork Orange, Eternal sunshine of the spotless mind, Imaginarium Of Doctor Parnassus, Angel-a, Enter the Void, Fight Club, Space Odissey,...

Oh, and I know it's not a movie, but, Doctor Who

Hey there! Lots of good films on these pages...It can be discussed what is really mindfuck, so just in case, here are some of my favorite "twisted" films:

Nowhere
Being John Malkovich
Lost Highway
Unlucky Monkey
Naked Lunch
A Tale of Two Sisters
2001: A Space Odyssey
Gummo

Paprika, Doom Generation The Wicker Man are other examples that are also pretty cool and odd but not quite up there with my favorites, untill I've rewatched them anyhow.

Also I don't think i've seen the most fucked up weirdo movies there is because in the end I need some sort of meaning/story behind it all, but I am very much up for recommendations ;)

I was thinking I'd start with El Topo and some more Cronenberg for starters, and perhaps some Svankmajer. What do you think? :)
